Novel Coronavirus Acknowledgement and Waiver

Please complete the attached form. I have filled in the program information so all you have to do is sign it. This form is required for players to be able to participate in workouts beginning today (Tuesday June 2). This supersedes the previous form. If you attended workouts last night and turned in the original form you are required to complete this form and turn it in prior to the next workout. Please let me know if you have any questions.
